Orem Help Center is located at the south end of City Hall
Please have your applications filled out and ready prior to your appointment. If applications or required items are incomplete, or you are 10 minutes late for your appointment, you will need to reschedule. All passports MUST be executed by staff by 4:30pm. Any incomplete applications will be turned away after that time and will need to be brought in a different time to be accepted.
what you Will need:
Bring a valid picture I.D. such as a driver’s license as well as your original birth certificate issued by the State or County Health Department.
Hospital issued birth certificates are not allowed.
The U.S. State Department requires that you pay their fee by check or money order with a separate check per application. Please make it out to “U.S. Department of State.”
You must pay the $35 City of Orem fee separately by cash, card, or check.

We can take your photo at our facility for $18 or you can bring one that meets passport requirements. Photos must have been taken in the last six months. For more information on photo requirements please go to: https://travel.state.gov/content/travel/en/passports/how-apply/photos.html
All applicants, including children, must be present at the time of the application submittal. If you are applying for a passport for a minor under the age of 16, both parents, as well as the applicant, must be present unless the parent has filled out a Statement of Consent (form 3053), and included a copy of the front and back of their ID.

You can now apply for routine processing service or pay an additional $60 for expedited service to receive your passport faster. For current processing time estimates, please visit travel.state.gov
- No white out, scribbles or cross out on application. Please complete new form.
- Any dates listed should always be in the format of mm/dd/yyyy (eg: 01/01/2022)
- Question #4 – Place of birth: must be City & State or City & Country
- Question #8 – Address Line 2: If applicant is a child, write “In Care of (insert parent’s name)” or just parent’s name
- Page 2 – Name of Applicant: last, first & middle
- Question #10 – Parental Information: Mother’s last name should be maiden name or last name at “parent’s birth”.
- Question #11 – Have you ever been married (if applicable): Name of spouse is listed as last, first & middle
- Question #18 – Travel plans: Write “none” if no travel plans
- Question #21 – Have you ever been issued a U.S. Passport Book or Passport Card? If yes, mark “submitting with application”.
